What are Risk Assessments and Care Plans?

Living with a neurological condition, brain injury, or neurodivergent diagnosis may mean that an individual is more vulnerable to risks due to the complexities of their condition. Everyday tasks that once seemed simple can become risky due to changes in physical, cognitive, or emotional abilities. If there is no safeguarding in place to manage these risks, the individual's safety, well-being, or ability to function independently may be put at risk.

Luckily, occupational therapists are highly skilled at performing risk assessments and care planning.

A risk assessment identifies any potential risks that may be present when an individual completes an activity. It may also identify any risks within an environment. The clinical analysis will provide clear steps and guidance, and will be accompanied by a detailed care plan that will support the individual's needs.

A care plan provides guidance to an individual, outlining the individual's specific needs around environments or completing activities. It also includes the goals of the individual, and the strategies and tools to achieve them. This plan will be accompanied by a risk assessment outlining the specific occupational therapy guidance.

An Occupational Therapist's Approach

Neurological occupational therapists are trained to assess, reduce, or eliminate risks. They take an evidence-based approach that considers:

  • The individual's participation, physical abilities, and body function
  • The individual's views, belief system, motivators, and goals
  • The individual's functional abilities and limitations
  • The potential for the physical and social environment to change, resulting in fluctuating risk
  • Where the individual has the insights into their difficulties to understand the risks

The care plan and risk assessment will advise on the risks that can be avoided within environments and activities, and the guidance on how to do so.

Risk Assessment and Care Plan Support

Krysalis neurological occupational therapists can also carry out risk assessments that relate to medication or finances, as well as ensuring home, educational, or work environments are safe. They may also identify necessary adaptations or suggest the use of aids and equipment to help the individual in daily life. Our risk assessments are suitable for all service streams, whether an individual's rehabilitation revolves around activities of daily living, work, or school.

Our neurological occupational therapists can also help family members and carers to understand these documentations, allowing for a more educated support network and a wider understanding of the individual's needs.
