Foundations of Success Assessment

The Foundations of Success Assessment explores the barriers that may be influencing success in a work role or in learning that may also be present in the home environment.

What is a Foundations of Success Assessment?

This expert assessment has been specifically designed to explore the needs of those who are suspected or confirmed to be neurodivergent. It included a standardised sensory assessment, and an evaluation of the environmental impacts on an individual.

This report clarifies how symptoms or traits influence well-being and affect function. For children and young people, this report will also clarify how these influences affect learning; for adults it will clarify how work performance is affected and will assess the suitability of the individual's role.

For our education dedicated service, the Foundations of Success assessment helps family members, case managers, and education professionals to understand the young person's needs and potential.

The remote (online) assessment examines sensory needs, environmental influences, and includes a personal impact analysis. The assessment seeks to establish what is required to promote success for the individual in everyday function and provides bespoke, person-centred recommendations for workplace adjustments/home and learning environments.

Who is a Foundations of Success Assessment for?

Working Age Individuals

Suitable for individuals of working age who are neurodivergent, whether they are present or absent from work.

Young Individuals

Suitable for a child or young person who is present or absent from education. Also appropriate for those transitioning through education or experiencing change.

Supporting Professionals

Appropriate for those seeking to understand what is needed for an individual to succeed in their work role, and how the environment influences their work capabilities and potential.

Family Members

Appropriate for family members seeking to understand what is needed to help a child or young person succeed in education, and how the environment influences their learning capabilities and potential.

What is Included in a Foundations of Success Assessment?

This assessment includes:

  • Review of medical status and current medical provision
  • Standardised impact and severity rating of neurological symptoms
  • Standardised sensory and environmental personal impact evaluation
  • Analysis of function with an emphasis on work routine, job role and demands/learner routine and demands including extra-curricular activities
  • Exploration of strengths, challenges and values, and motivators relating to work/learning
  • Adjustment and support recommendations
  • Recommendations regarding alternative roles (dedicated work service only)

This assessment can be undertaken in-person by the occupational therapist if this can be tolerated by the individual. Additional charges will apply.

The Outcomes of your Assessment

Upon completion of the Foundations of Success assessment, the recommendations found within the assessment report will enable an employer or education professioinal to take steps towards positive change that will lay the foundations for an individual's success in their work role, or in their learning environment.

This report will be able to provide valuable insights of an individual's strengths and challenges, and how these strengths and challenges impact the individual's capabilities and well-being.

The neurological occupational therapist will be able to provide recommendations for the individual, their employer, or the educational professional on how the strengths and challenges can be managed or supported.

For those receiving a Foundations of Success Assessment via our work dedicated service, the occupational therapist will also make recommendations regarding alternative roles for the individual.
